Women’s Endocrine Health Center (WEHC) brings family physicians and specialists together to improve coordinated care for women with complex endocrine conditions. Using a Value‑Based Healthcare model, the team developed workshops, digital PROMs, updated workflows, and new referral pathways. Team is working on integrated digital tools and improved communication processes to enhance patient experience and inform future clinical program development at PHC
